Dr. Brian Trieu-Mallari, PT, DPT, NCS

Founder and Owner of Homestretch Neuro Physical Therapy

My name is Brian Trieu-Mallari, Doctor of Physical Therapy and Board-Certified Neurologic Clinical Specialist. I received my doctorate in PT from Samuel Merritt University in 2018 and went on to complete my neurologic residency training at Kaiser Permanente in 2020. From outpatient clinics to trauma centers, rehab hospitals to family homes, I’ve gained experience in all stages and settings of recovery, culminating in my creation of Homestretch Neuro Physical Therapy.

I’d like to say thank you for visiting Homestretch. Working with people with neurologic injuries or conditions has always been a unique privilege for me, but there’s something incredibly special about working with people in settings that are meaningful to them. Homestretch Neuro Physical Therapy is my way of maximizing treatment potential and delivering the best of my services and skills directly to you. Whether your goal is to relearn how to snowboard, walk your niece to school safely, or stand up on your own, I’m here to help you reach every big and little milestone.

Outside of Homestretch, I love to explore new places to eat, play music, hike with my dog, train for the next marathon and go on travel adventures with my wife.

Dr. Thanh Bach, PT, DPT, NCS

Bach is a licensed physical therapist with a Doctorate in Physical Therapy. He began his career after graduating from Samuel Merritt University in 2024 and completing a year-long, neuro-focused residency at Kaiser Foundation Rehabilitation Center in Vallejo, CA.

Bach focuses on individuals recovering from strokes, Parkinson’s, brain and spinal cord injuries, orthopedic challenges, and those living with chronic pain. His approach is collaborative and client-focused, with care plans tailored to each person’s unique goals and lifestyle.

Much of Bach’s experience is in functional rehabilitation, where progress can be both fast-paced and rewarding. He also has a growing interest in expanding the role of physical therapists beyond the traditional insurance-based model, with a focus on supporting overall health and long-term wellbeing.

Outside of work, Bach enjoys exploring the city on foot or by bike, often discovering new cafés and bakeries while listening to podcasts on topics ranging from history and mystery to business/finance and cultural phenomena. He is also a dedicated San Francisco Giants fan and Los Angeles Dodgers hater.

Dr. Masato Kobayashi, PT, DPT, NCS

Masato is a neurologic physical therapist with a Doctorate of Physical Therapy from Western University of Health Sciences. He has experience with a wide variety of conditions including stroke, brain injury, spinal cord injury/disease, vestibular disorders, neuromuscular disorders (i.e. Guillain-Barre Syndrome), inflammatory disorders (i.e. multiple sclerosis), and degenerative diseases (i.e. Parkinson's Disease). Additionally, Masato has advanced training in vestibular rehabilitation through Duke University and the American Physical Therapy Association, and he also serves on a dedicated spinal cord injury multidisciplinary team. He is especially passionate about helping individuals with spinal cord injuries regain independence and quality of life.

By integrating evidence-based practice with individualized, goal-driven care and high-intensity training, Masato is able to help his clients realize their true capabilities. He is committed to empowering clients through education, self-advocacy, and high-quality rehabilitation so they can confidently return to the activities that matter most.

Masato is a former dancer which influences his understanding and coaching of human movement. In his spare time, he enjoys working out, spending time with friends, and eating at new restaurants with his spouse. His 2026 goal is to pick up more hobbies outside of work - he is currently learning photography in hopes to capture more memories on his international travels.
